This is a medicine review service available if you have been diagnosed with Type 2 Diabetes. How can a Diabetes Medicine Review help?

A Diabetes Medicine Review allows you to better understand why you are taking the medicines you have been prescribed and gives you the opportunity to talk to a pharmacist about any problems you might have with your medicines. It's important that patients understand how to take their medicines, possible side effects to look out for and how to store your medication. At the end of the consultation, you will receive a list of your current medicines with an action plan if needed. With your consent the pharmacist can share this information with your GP so they can work together with you to get the best outcome for your health.

Am I eligible for a Diabetes Medicine Review?

You can receive this service if: you are a Medicare and/or Department of Veterans’ Affairs (DVA) cardholder; have not received a Medicines Review, Diabetes Medicines Review, Home Medicines Review (HMR) or Residential Medication Management Review (RMMR) in the last 12 months; are living at home in a community setting; are unable to gain timely access to existing diabetes education/health services in your community; have recently been diagnosed with type 2 diabetes (in the last 12 months); have less than ideally controlled type 2 diabetes.

What is the cost for a Diabetes Medicine Review service?

This service is available for free should you meet the eligibility criteria. Please check with the pharmacist to see if you meet the criteria. The eligibility criteria can be found in ‘Am I eligible for a Diabetes Medicine Review?’ above.

What should I bring to my appointment?

Your consultation will take place in the pharmacy with a qualified Pharmacist. Please bring (i) all your medicines including any over the counter medicines, eye drops, creams, herbal medicines, and vitamins (ii) a list of your medicines that you are currently taking if one exists (iii) a list of questions you would like to ask the pharmacist about your medicines (iv) any blood pressure or blood glucose readings you may have recorded.

What does the consultation involve?

At the consultation the pharmacist will obtain your consent prior to providing the service. The intention of the Diabetes Medicine Review is to review your medicines to obtain a complete picture of how you are managing your health in order to create an action plan with agreed goals if necessary. The pharmacist will prepare a medication profile and provide you with a Medicines Review Report you can share with your GP.
